Applying for services at CVI is your first step towards independence. For your convenience, you can initiate the process by filling out the application and medical release form.

To complete the application via telephone or to find out if you are eligible for Medicare coverage, contact an intake counselor in the Client Services Department at 404-875-9011.
CVI Monthly Open House - Come and See What CVI Can Do for You
Every 4th Thursday, 1:00 p.m. - 2:00 p.m.
Are you or someone you know having difficulty reading the newspaper or small print? Have you recently experience a reduction in your vision that hinders your daily tasks? Are you concerned about loss or gain of employment due to your decrease in vision? Do you have challenges watching TV or seeing your computer screen? Do you want to learn how to be more independent in your home?
If you or someone you know is visually impaired or blind, we invite you to attend our monthly Open House. For one hour prospective clients and their families will learn about the Center’s services and programs, complete a short application for services and get a tour of the facility. All Open Houses’ are held on the 4th Thursday of the month from 1 p.m. until 2 p.m. in the Bellsouth conference Room at 739 West Peachtree Street, NW.
Sign in starts at 12:45 p.m.; and we start promptly at 1 p.m. We are located in the Midtown area of Atlanta so please allocate extra time for travel and parking.
